To use it, first create a signature subpage under the name User:Your Username/sig and then go to your preferences. In the nickname/signature field, insert the following code:
{{SUBST:nosubst|User:Your Username/sig}}
Make sure the "treat signature as wikitext" box is ticked, and save the change at the bottom of the preferences page. You can now use ~~~~ after your comment to sign it using your template, with a timestamp.
